Back to School for New Families

Everything you need to know in anticipation of the 24-25 school year at OSM!

Enrollment Documentation

The following documentation must be submitted in order for your child to be fully enrolled. Documentation can be emailed to enrollment@osmschool.com or physical copies can be dropped off at the front office.

  • Birth Certificate (should have been submitted alongside your application)

  • Current immunization report

  • Dental records

  • Vision records (K-8 Only)

  • Allergies (if needed)

  • Medications (if needed)

  • Home Language Survey

  • Pastoral Intake Form

    • If children received sacraments at OSM or have previously sent in sacramental records, they do not need to resend them. 

    • This form is for all families, not only those who have children receiving sacraments.

  • Baptismal Certificate (if you have one from any faith denomination, we will need it to be submitted)

  • First Communion Certificate (required for Catholics only, 2nd grade and up)

  • Please include any court orders or documents pertaining to your child’s custody in regards to parent pickup (if needed)

Parent Volunteer Forms

In order to volunteer at OSM, parents need to physically sign and submit the following forms:

Uniform Information

Uniforms can be purchased new at Big Kahuna, located at 3509 S Halsted St.

Our uniform resale for the 24-25 school year will be held on Friday, August 2nd from 8am-1pm and Sunday, August 4th from 9am-12pm in the Parish Center. 

Extended Day

Details for Extended Day can be found HERE. We will announce Enrichment Club sign-ups in early September. If you would like to register your child for Extended Day, we will have a registration link in the first Sunday Update of the school year. If you know your child will be attending regularly, you can let us know and they will be added to the permanent list.

Save the Date! You're Invited to Oktoberfest!

SAVE THE DATE - 15th Annual Oktoberfest on Saturday, October 5 at 6PM
 
MARK YOUR CALENDARS:  Family, friends, parishioners, & alum of OSM are invited to the 15th Annual OSM Oktoberfest! This OSM tradition is a fun-filled evening for adults complete with food, drinks and entertainment. More details are coming soon!

SIGN UP TO VOLUNTEER: Community events at OSM are parent-led, so get a jump start on your volunteer hours and CLICK HERE to sign up for the Oktoberfest planning committee. Help is needed with securing donations, event setup, volunteering during the event, etc. Please complete the form no later than Friday, July 26.

CALLING ALL NEW OSM PARENTS: CLICK HERE to sign up now for our New Parent Meet & Greet which will take place one hour before Oktoberfest at neighborhood favorite Weather Mark Tavern. But hurry, secure your spot now because space is limited!
