ABSENCE FORM

For each day of absence due to illness, please complete each box. 

 

All absences must be submitted before 7:45am.

If the absence if not illness related, please complete the section requesting all absent days. 

If a parent or guardian fails to report an absence, it will be recorded as an Unexcused Absence.

Medical absences longer than three days may require medical documentation to return.

 

This form will go to both the Attendance and OSM Health emails. You may email teachers separately.
